Role Overview:
Provides direct patient care activities within a home environment, including assessment, treatment planning, and supervision of physical therapy practices.
Key Responsibilities:
- Instructs and assists patients with physical activities and use of supportive devices.
- Educates patients and families on treatment procedures.
- Re-evaluates patients and adjusts treatment plans for optimal outcomes.
- Collaborates with physicians and healthcare practitioners to coordinate care and modify treatment as needed.
- Evaluates, fits, and adjusts prosthetic and orthotic devices, recommending modifications.
Qualifications:
- Bachelor’s Degree in Physical Therapy (PT).
- Valid Driver’s License and Physical Therapist License.
- No prior experience required.

- Ability to adapt treatment plans based on patient progress.
- Strong collaboration skills for interdisciplinary coordination.
