Role Overview: Provides direct patient care within a 9-bed, open 24-hour unit, collaborating with PT, OT, and Speech therapists to support patient recovery and functional restoration.
Responsibilities include:
- Promoting patient and family-centered care, including education on illness management, post-treatment home care, and medication administration.
- Participating in activities that ensure patient safety, quality improvement, and regulatory compliance.
- Developing, implementing, and documenting personalized care plans with clear goals, ensuring effective communication across care teams and with patients and families.
- Assessing and evaluating physical, psychosocial, and emotional needs, considering patient preferences, values, and cultural factors.
- Utilizing critical nursing skills to provide comprehensive care in a safety-sensitive environment.
Qualifications:
- Minimum: Nursing Diploma/Associate’s Degree and RN license.
- Preferred: Bachelor’s Degree in Nursing and at least 2 years of experience.
