Role Overview: Provides direct patient care activities, including assessment, diagnosis, planning, implementation, and evaluation, within established nursing standards. Key Responsibilities:
- Develops and documents individualized care plans with clear goals, collaborating with the healthcare team and patients/families.
- Assesses physical, psychosocial, and emotional needs, respecting patient preferences, values, cultural, and spiritual considerations.
- Ensures effective communication across care continuum and proactively addresses barriers to involvement.
- Maintains safety standards and adheres to regulatory compliance.
Qualifications & Skills:
- Education: Nursing Diploma/Associate’s Degree required; Bachelor’s Degree preferred.
- Licenses & Certifications: Valid RN license.
- Experience: No prior experience required; 2+ years of nursing experience preferred.
- Skills: Critical thinking, patient education, communication, teamwork, and safety awareness.
