Role Overview: The Physical Therapist provides primary rehabilitative care in patients’ homes, managing 25-35 individuals, and supervises PTAs and Home Health Aides. They develop and coordinate personalized care plans, ensuring goals are met through collaboration with multidisciplinary teams including physicians, nurses, social workers, and caregivers.
Key Responsibilities:
- Assess patient needs and perform routine and complex therapies based on physician orders.
- Coordinate services throughout the episode of care, adjusting care plans as needed.
- Supervise and review care provided by support staff, ensuring quality and compliance.
- Educate patients and caregivers to promote independence and self-care.
- Communicate patient status changes to physicians and participate in team conferences.
- Ensure documentation complies with regulatory standards, including CMS and Joint Commission guidelines.
- Participate in performance improvement initiatives and ongoing professional development.
- Meet productivity standards and perform additional duties as assigned.
Qualifications:
- Graduate of an accredited physical therapy program.
- Valid Physical Therapist license and BLS certification.
- Georgia Driver’s License and private vehicle insurance.
- Minimum of 1 year full-time physical therapy experience; home health experience preferred.
Skills: Strong assessment, rehabilitation, leadership, communication, organization, and time management skills. Proficiency with Windows, home health software, and understanding of regulatory processes is essential. OASIS knowledge is preferred.
