Physical Therapist PRN

Role Overview: A physical therapist assesses, plans, and implements rehabilitative programs to improve patient mobility, relieve pain, and prevent deformity. Services are delivered across the full continuum, emphasizing quality, cost-efficiency, and regulatory compliance.


Key Responsibilities:

  • Patient Assessment: Collaborate with physicians to develop and evaluate treatment plans, goals, and discharge strategies, involving patients and families.
  • Treatment Planning and Care: Execute treatment plans, select appropriate activities, modify goals as needed, and provide education and discharge instructions.
  • Documentation: Maintain accurate records of evaluations, treatment sessions, family education, and discharge details per departmental protocols.
  • Professional Conduct: Uphold ethical practices, maintain confidentiality, punctuality, dress code adherence, effective communication, and participate in meetings and training.
  • Customer Service: Deliver compassionate care, apply AIDET principles, foster teamwork, and promote a positive environment.
  • Professional Development: Engage in ongoing education, complete competencies, participate in orientation, and meet licensure requirements.

Qualifications & Skills:

  • Masters in Physical Therapy required; Doctorate preferred.
  • Licenses: PT license and BLS certification upon hire.
  • Minimum one year of experience in rehab, LTACH, acute care, or SNF settings.
  • Effective communication, customer service focus, teamwork, and basic computer skills.

Additional Requirements: Perform other duties as assigned and comply with all policies and standards.
