Role Overview: Provides evaluation and treatment for patients primarily in Rehab and LTAC hospitals, focusing on improving patient function and well-being. Responsibilities include conducting initial and discharge assessments, developing and implementing treatment plans, educating patients, families, and staff, and documenting progress in the EMR. Collaborates with the interdisciplinary team, communicates patient updates, and manages equipment orders and vendor contacts. May serve as a mentor for interns and participate in skills checks.
Qualifications & Skills:
- Bachelor’s Degree in Physical Therapy required; Doctoral Degree preferred.
- GA Physical Therapy license or eligibility; BLS certification from the American Heart Association.
- Minimum of 1 year experience in Rehab, SNF, LTACH, or Acute settings preferred.
- Basic computer literacy.
