Primary Care Physician-OSH – Live Well Homecare

Role: Primary Care Physician
Responsibilities:

Assess, diagnose, and oversee primary care for patients

Prescribe medications and treatments

Maintain Electronic Medical Records

Collaborate with leadership to meet healthcare quality goals

Manage and direct care teams with Practice Managers

Contribute to delivering equitable, value-based healthcare focused on outcomes and preventive medicine

Perform other duties as assigned

Qualifications & Skills:

Medical Doctor (MD) or Doctor of Osteopathy (DO) with Board Certification or eligibility in Internal Medicine or Family Medicine
Active, unrestricted State Medical License and DEA license
US work authorization
Preferred: Geriatrics fellowship, experience with older adults, value-based care, outpatient primary care, bilingual skills

Additional Details: This full-time role involves working approximately 40 hours weekly, with compensation ranging from $174,070 to $374,920, depending on experience and location.
