Role Overview: This position involves providing bedside nursing care as a Hospice Homecare Case Manager Registered Nurse, focusing on delivering compassionate post-acute services, including hospice, home health, and rehabilitation. The role emphasizes collaboration to improve clinical outcomes, support patients’ needs, and ensure quality care delivery.
Responsibilities:
- Assess patient conditions and develop individualized care plans.
- Coordinate and communicate with interdisciplinary teams, patients, and families.
- Administer treatments, monitor patient progress, and document care activities.
- Ensure compliance with healthcare standards and safety protocols.
- Provide education and emotional support to patients and families.
Qualifications & Skills:
- Current RN license with at least one year of experience in healthcare.
- Basic Cardiac Life Support certification within 30 days of employment.
- Valid driver’s license within 30 days of employment.
- Strong communication, organizational, and compassionate care skills.
