Epilepsy Nurse Navigator (RN)

Role: The Nurse Navigator (NN) is a licensed registered nurse specializing in disease-specific care, providing individualized support to patients, families, and caregivers to improve access, education, and care coordination throughout the cancer continuum.


Responsibilities:
  • Serve as subject matter expert in navigation, promoting timely access to care and patient-centered services.
  • Collaborate with program leaders, perform data evaluations, and support program development based on community needs.
  • Perform nursing processes including assessment, diagnosis, planning, and evaluation, ensuring individualized, evidence-based care.
  • Coordinate multidisciplinary care, facilitate communication, and identify service gaps for optimal patient outcomes.
  • Support performance improvement, participate in professional development, and mentor other nurses and staff.
  • Ensure compliance with organizational policies, safety standards, and legal requirements.

Qualifications:
  • Bachelor’s or Master’s Nursing degree preferred.
  • Current RN license with appropriate certifications (BLS, ACLS) upon hire.
  • National certification in a disease-specific area within one year.
  • Minimum 5 years of nursing experience, with at least 2 years in disease-specific outpatient care or care coordination.
