We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities, utilizing our Geriatric House Calls™ model.
Responsibilities include conducting evaluations, developing personalized treatment plans, delivering evidence-based therapy to improve mobility, independence, and quality of life, and documenting in our EMR system.
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists, and educate patients, caregivers, and staff to promote safety and aging in place.
The role offers flexibility to incorporate telehealth and virtual care, allowing clinicians to create and manage their own schedules and support more patients.
Qualifications include a valid state PT license, degree from an accredited program, basic computer skills, CPR certification, and physical ability to meet the demands of in-home care, including lifting and mobility assistance.
