Physical Therapist

Role Overview: A licensed Physical Therapist responsible for providing one-on-one geriatric physical therapy services in patients’ homes and senior living communities. The role involves conducting evaluations, developing individualized treatment plans, delivering evidence-based therapy, and educating patients and caregivers to promote safety and independence.


Key Responsibilities:

  • Deliver personalized therapy sessions supporting mobility, longevity, and functional independence
  • Document care effectively within electronic medical records
  • Collaborate with an interdisciplinary team of healthcare professionals
  • Incorporate telehealth and virtual care into treatment plans
  • Assist with patient transfers and perform physical assessments as needed

Qualifications & Skills:

  • Valid Physical Therapy license and degree from an accredited program
  • Proficiency in computer and EMR systems
  • Current CPR certification

Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, and drive between locations while adhering to safety protocols.
