Role Overview
Provide comprehensive in-home health assessments to adult and geriatric patients, reviewing medical history, current health, medication adherence, and social factors. Conduct assessments lasting up to one hour, collaborating with healthcare providers, social workers, and family members to improve care quality and identify undiagnosed conditions.
Responsibilities
- Perform medical history review, diagnosis, treatment planning, health education, and referrals
- Administer point-of-care and lab testing as needed
- Document thoroughly following protocols
- Coordinate with physicians, case managers, and caregivers
- Deliver services at members’ residences, including homes and Skilled Nursing Facilities
Qualifications & Skills
- Master’s Degree or equivalent experience with NP licensure and prescriptive authority
- Board certification in Adult, Gerontology, Family, Emergency, or Acute Care
- Minimum one year NP experience
- Current BLS, ACLS, or CPR certification
- Ability to travel extensively, including overnight and weekend stays
- Valid driver’s license and insurance in good standing
- Strong communication, computer skills, and familiarity with medical records software
