Role: Integrated Care Coordinator responsible for managing complex, high-risk older adult members with medical, behavioral, and socioeconomic needs.
Responsibilities:
- Serve as the primary care manager, developing individualized, person-centered care plans.
- Conduct comprehensive assessments covering medical, behavioral, functional, cultural, and social determinants of health.
- Collaborate with care teams, providers, and community resources to implement care plans.
- Provide referrals and linkage to internal and community-based services.
Qualifications & Skills:
- Master’s in Social Work or Bachelor’s in Social Work, Psychology, or related field with 1+ years case management experience.
- 2+ years supporting older adults or with long-term support services.
- Experience with behavioral health providers and community resources addressing SDoH.
- Proficient in MS Office, with reliable transportation and Indiana residency.
Additional Requirements:
- Ability to travel up to 75%, work face-to-face and telephonically, and navigate multiple locations.
- Preferred: certifications in Motivational Interviewing, Trauma-Informed Care, bilingual skills, and experience in team-based care or managed care.
