Role: Provides speech therapy services to patients with speech, language, or swallowing disorders in a home health and hospice setting.
Responsibilities include:
- Assessing and documenting patient speech, language, and swallowing data.
- Monitoring changes in physical and emotional health parameters.
- Reviewing and modifying treatment plans based on individual performance and needs.
- Instructing patients on monitoring speech and practicing new skills.
- Maintaining compliance with legal, organizational, and regulatory documentation requirements.
- Performing additional job-related duties as assigned.
Qualifications and skills:
- Master’s Degree or foreign equivalent in Speech-Language Pathology.
- Licensure as a Speech Pathologist in the practice state.
- CPR or BLS certification from an approved provider.
- Valid Driver’s License and age of at least 21.
- Preferred: ASHA Certificate of Clinical Competence (CCC).
