Physical Therapist (PT)

Role Overview: The Physical Therapist (PT) provides direct rehabilitation care to patients in a skilled nursing facility setting, focusing on long-term support and short-term rehabilitation for diverse patient populations with orthopedic, neurological, and cardiopulmonary needs.

Key Responsibilities:

  • Develop and implement personalized treatment plans based on physician referrals.
  • Assess and modify goals to ensure measurable, functional progress.
  • Supervise and evaluate the care provided by PT assistants, aides, and students.
  • Maintain accurate documentation and utilize therapy software effectively.
  • Collaborate with interdisciplinary teams and communicate with patients, families, and support staff.
  • Adhere to safety, confidentiality, and professional standards, including compliance with laws and regulations.
  • Participate in community health initiatives and demonstrate excellent customer service.
  • Ensure proper body mechanics and safety during patient care activities.
  • Qualifications & Skills:

  • Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT).
  • Current licensure in applicable state, maintained in good standing.
  • Preferred one year of experience; new graduates are welcome.
  • CPR certification required or obtained during orientation.
  • Proficiency in Microsoft Office Suite and effective communication skills.
  • Ability to make independent decisions, demonstrate professionalism, and work effectively in a team environment.
