We are seeking a Home Health Physical Therapist to provide personalized in-home physical therapy aimed at improving patients’ strength, mobility, and independence.
The role involves
Planning and administering prescribed treatment plans in patients’ homes. Assessing and documenting physical strengths to support clinical evaluation. Developing and implementing rehabilitation programs aligned with physician plans and patient goals. Providing clear instructions to Physical Therapist Assistants and Home Health Aides, overseeing their work and documentation. Documenting patient progress, interventions, and evaluations thoroughly and promptly. Reporting patient progress to physicians, care teams, patients, and families, and submitting required documentation.
Qualifications and skills include a current, unrestricted Physical Therapy license, experience collaborating with healthcare professionals, and at least one year of relevant experience.
