Role: Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults.
Responsibilities: Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapy to improve mobility and independence, document within EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills: Valid PT license, degree from an accredited program, computer proficiency, CPR certification, and ability to meet physical demands such as lifting, repositioning, and driving.
Key Duties: Deliver personalized one-on-one care in various settings, support patient wellness, promote aging in place, and maintain effective documentation.
