Role: A Part Time Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations to help improve patient health outcomes.
Responsibilities: Conduct 45-minute visits in patients’ homes or virtually, review medical and medication history, perform physical evaluations, and administer diagnostic tests as ordered. Build trusted relationships, deliver personalized healthcare, and ensure patient comfort and safety in diverse home environments.
Qualifications & Skills: Active, unrestricted licensure, board certification, multi-state licenses preferred, and ability to cover primary locations. Strong communication skills, compassion, flexibility to travel locally and within licensed states, and proficiency with clinical workflows and devices.
