Role: Night Shift Licensed Practical Nurse (LPN)
Responsibilities:
- Provide resident-centered nursing care in long-term care and post-acute settings
- Administer medications and treatments accurately
- Monitor resident health status and document in EMR systems
- Support admissions, discharges, and care planning within an interdisciplinary team
- Mentor Certified Nursing Assistants (CNAs) and promote dignity, safety, and comfort
Qualifications & Skills:
- Valid LPN license
- Compassionate bedside care skills
- Ability to work collaboratively within a team
- Strong documentation and observation skills
