Role Overview: The RN Healthcare Manager supports high-risk patients by providing assessment, education, and advocacy to improve their quality of life.
Key Responsibilities: Identify high-risk patients, manage chronic conditions through assessments, medication reconciliation, motivational interviewing, and referrals. Facilitate transitions from inpatient to outpatient care, coordinate care plans, document accurately in electronic records, participate in team meetings, and perform clinical tasks within licensure scope.
Qualifications: Bachelor’s or Associate’s degree in Nursing with 4+ years of experience, active RN license, current BLS, and 2+ years in clinical or hospital settings. Knowledge of chronic diseases like COPD, asthma, diabetes, CHF, and IHD. Proficient in electronic medical records.
