Role Overview:
Seeking a registered nurse to provide in-home assessments, patient education, and care coordination in a community-based setting.
Responsibilities include evaluating members’ health, identifying treatment barriers, making referrals, and supporting self-management.
Maintain communication with members through in-person visits or phone interactions to ensure quality care continuity.
Qualifications & Skills:
- Unrestricted RN license in Washington
- Basic Life Support certification or ability to obtain
- At least 1 year of clinical and home care experience
- Strong computer skills, customer service, patient-focused attitude, and adaptability
- Valid driver’s license and reliable transportation
Preferred Qualifications:
- BSN or CCM certification
- Experience in discharge planning, utilization review, or risk management
- Telephonic care experience and bilingual in English/Spanish
- Proficiency in MS Office and managed care background
