Seeking a Registered Nurse (RN) Home Health professional to provide skilled nursing services and coordinate home care for patients in the San Antonio area. Responsibilities include performing nursing assessments, developing and evaluating care plans, administering medications, wound care, and therapeutic treatments. The role involves patient education, health promotion, and disease prevention, ensuring quality and safe care in compliance with regulations. The nurse will evaluate patient conditions during visits, communicate with healthcare team, manage care activities, and document all interactions. Position requires traveling within designated areas, with shifts from Friday to Monday, and includes on-call duties, mileage reimbursement, and shift differentials.
Qualifications include:
- Graduate of an accredited nursing school
- Current unrestricted Texas RN license or compact license
- BLS and IV certification
- At least 1 year of home health or related experience
- Wound care expertise
- Proven computer skills
- Ability to move and transfer patients
- Reliable transportation and valid driver’s license
- Proof of immunity to certain communicable diseases or vaccination documentation
