Per Diem Hospice & Palliative Care Nurse Practitioner

Role: Nurse Practitioner providing palliative care consultation services in various settings, including nursing homes, hospitals, and community environments.


Responsibilities:

  • Implement specialist palliative care consultations, including history-taking, examinations, care planning, and communication with patients and families.
  • Maintain effective communication with referral sources and ensure accurate, compliant documentation.
  • Develop quality initiatives, educate staff, and support hospice operations through participation in on-call rotations.
  • Perform clinical and administrative tasks within professional scope, demonstrating professionalism and respect.
  • Travel within assigned areas to deliver patient care and coordinate interdisciplinary resources.

Qualifications & Skills:

  • Master’s degree in Nursing and graduation from an approved Nurse Practitioner program.
  • NY State RN licensure and certification as an Adult, Family, Gerontology, or Palliative Care Nurse Practitioner.
  • Preferred: 1+ year clinical experience, hospice or palliative care background, CPR-BLS certification, and hospice & palliative care certification.
  • Proficiency in Microsoft Word and Excel, strong communication, listening, organizational, and critical thinking skills.
  • Ability to work flexibly, travel within the geographic area, and collaborate effectively within an interdisciplinary team.
