Social Worker LCSW – Social Work Services

Role Overview:

The Licensed Clinical Social Worker (LCSW) provides comprehensive biopsychosocial/spiritual assessment, diagnosis, treatment, and advocacy for patients and their support systems. They collaborate with patients and multidisciplinary teams to develop and implement evidence-based, person-centered care plans across the care continuum, respecting patient self-determination and reducing care barriers. The role involves working with inpatient and outpatient populations, adhering to ethical standards and organizational mission.

Responsibilities include:

  • Conducting assessments and developing care plans
  • Collaborating with healthcare teams
  • Supporting diverse patient populations
  • Promoting equitable care and respecting patient autonomy
  • Qualifications and Skills:

  • Master’s Degree in Social Work from an accredited or CSWE-candidate program
  • Licenses in applicable states (Oregon or Washington) upon hire
  • At least 2 years of relevant social work experience
  • Strong collaborative, communication, and ethical practice skills
