Role: Per Diem Orthopedic Home Physical Therapist
Responsibilities:
Evaluate and treat patients in their homes, focusing on orthopedic rehabilitation, including post-operative care for hip and knee replacementsEducate patients on safe mobility, transfers, and ambulation, ensuring safety for dischargeDevelop and modify individualized treatment plans based on evaluations and goalsCollaborate with interdisciplinary teams and support systemsDocument care, progress, and discharge plans accurately following regulatory and departmental guidelinesPerform functional training, including ADLs/IADLs, therapeutic activities, and community reintegrationEnsure safe, effective interventions and maintain compliance with policiesUtilize EMR software for documentation and billingParticipate in professional development activities such as billing and coding seminarsQualifications & Skills:Licensed Physical Therapist with a Master’s or higher degree; Doctorate preferredStrong orthopedic background, especially in total hip and knee replacementsExcellent communication, clinical decision-making, and computer skillsAbility to work independently and as part of a team with professionalism and dedicationExperience with EMR software and understanding of reimbursement systems is preferredKey Traits: team-oriented, motivated to make an impact, committed to patient success and growth opportunities.
