Seeking a dedicated Physical Therapist to provide high-quality therapy services in patients’ homes in a mobile outpatient setting.
Responsibilities include:
- Assessing patients’ physical conditions and developing personalized treatment plans.
- Implementing evidence-based therapies to promote mobility, independence, and wellness.
- Working directly with patients in their homes, primarily 1-2 days a week.
- Managing Part B–only documentation, resulting in less charting and more patient interaction.
