We are seeking a Home Health Speech Language Pathologist (SLP) to provide personalized speech therapy services in patients’ homes, focusing on communication, swallowing, and cognitive impairments resulting from illness or injury.
Key responsibilities include:
- Conducting assessments, documenting progress, and ensuring patient goals are met
- Implementing therapy exercises for speech, swallowing, and cognition
- Educating patients and families on care plans, safety, and self-care practices
- Collaborating with a multidisciplinary home health team to deliver comprehensive care
Qualifications and skills required:
- Master’s or Doctoral degree in Speech Language Pathology and active license in Colorado
- At least one year of experience in speech therapy, preferably in home healthcare
- CPR certification and ability to pass background and drug tests
- Knowledge of home health regulations, strong clinical judgment, empathy, and excellent communication skills
