Role Overview:
The Physical Therapist (PT) is responsible for providing home health physical therapy services to patients, facilitating recovery in their own homes. Key duties include conducting assessments, developing and modifying plans of care, delivering therapy per physician orders, and documenting patient progress to ensure optimal outcomes. The PT collaborates with healthcare team members, educates patients and caregivers on home exercise programs, and supervises other personnel such as PT aides or home health aides where applicable. They also complete necessary assessments like OASIS, ensure compliance with policies, and maintain effective communication with staff and families.
Qualifications & Skills:
- Degree in Physical Therapy from an accredited program
- Current, unrestricted state license
- Valid CPR, driver’s license, and proof of auto insurance
- Preferred: strong organization, communication, attention to detail, time management, and experience in home health therapy and OASIS documentation
Key Duties:
- Perform assessments and interpret tests to set functional goals
- Develop, implement, and revise individualized plans of care
- Deliver therapy services and document progress effectively
- Coordinate with healthcare team and educate patients/caregivers
- Supervise relevant personnel and complete required assessments within scope of practice
