We are seeking a full-time Case Manager to support our Emergency Department team, focusing on discharge planning, patient resource connection, and promoting positive outcomes.
Responsibilities include: coordinating care from pre-admission to post-discharge, facilitating communication among healthcare teams and community agencies, and advocating for patients. For Social Workers, duties involve crisis intervention, understanding human behavior, responding to abuse or neglect, and developing care plans with appropriate referrals. For Care Managers, responsibilities encompass care integration, improving patient flow, and evaluating outcomes based on clinical, financial, and utilization data.
Qualifications: a Bachelor’s Degree in Nursing or Social Work, RN license in Minnesota, and Basic Life Support certification (for social workers within six weeks).
