Join a team as a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant)
Responsibilities include:
Conducting comprehensive in-home health assessments, including medical history review, physical exams, and screenings
Identifying and documenting diagnoses for care management
Communicating findings with primary care providers to address care gaps
Recognizing urgent situations and intervening appropriately
Educating members on health conditions, medications, and adherence
Addressing social determinants of health and providing referrals as needed
Qualifications:
Active and unrestricted licensure in the practicing state
Certification relevant to the role (ANCC, AANP, NCCPA)
Valid driver’s license and reliable transportation for home visits
Ability to navigate varied home environments and perform assessments
Preferred: 1+ years of clinical experience, proficiency with electronic records, and strong communication skills with diverse populations
