Role Overview:
As an Advanced Practice Clinician in a home-based care program, you will perform comprehensive in-home health assessments for Medicare Advantage and other plan members.
You will focus on improving health outcomes through patient education, identifying care gaps, and collaborating with primary care providers, without prescribing medications.
This role offers flexible scheduling, no on-call, weekends, or holidays, promoting work-life balance.
Your primary duties include conducting physical exams, reviewing medical histories, documenting diagnoses, communicating with PCPs, recognizing urgent situations, and addressing social determinants of health.
You should hold active licensure, relevant certifications, a valid driver’s license, and have reliable transportation.
Preferred qualifications include clinical experience in family, geriatric, or home health care, proficiency with electronic records, and strong communication skills.
Compensation ranges from $109,500 to $164,000, plus benefits, incentives, and performance-based rewards.
