Role: HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant)
Responsibilities:
- Conduct comprehensive in-home health assessments, including medical history, medication reconciliation, vital signs, and physical exams
- Identify, document, and communicate diagnoses to primary care providers for care management
- Perform evidence-based screenings and point-of-care testing as needed
- Recognize urgent situations and intervene appropriately
- Educate members on health conditions, medications, and adherence
- Address social determinants of health and provide referrals
Qualifications & Skills:
- Active certification (ANCC, AANP, NCCPA) and unrestricted state licensure
- Valid driver’s license and reliable transportation for home visits
- Ability to navigate varied home environments and perform assessments requiring sitting, standing, or kneeling
- Preferred: 1+ years clinical experience, proficiency with electronic medical records, strong communication skills with geriatric or Medicare populations
