Role Overview:
This position involves providing in-home medical care within assigned counties, collaborating with interdisciplinary teams to support patient health and well-being. Responsibilities include conducting comprehensive assessments, developing and updating care plans, performing basic nursing procedures, and educating patients and their families on disease management and wellness. The role requires coordination with healthcare providers, follow-up assessments, and participation in quality improvement initiatives, ensuring high standards of care. The provider must practice according to state laws and maintain current medical knowledge through continuing education.
Qualifications & Skills:
- Current RN/NP license or ability to obtain it
- Certification from AANP or ANCC
- Active DEA licensure or ability to obtain
- Reliable transportation and valid driver’s license
- Preferred: 2+ years clinical NP experience in primary or urgent care settings, strong organizational and communication skills, and computer literacy
