Seeking an experienced Primary Care Advanced Practice Clinician (NP or PA) to independently manage a patient panel in a collaborative, value-based care setting.
Key responsibilities include:
- Providing comprehensive, longitudinal primary care, including assessments, diagnoses, and treatment.
- Managing acute and chronic conditions, care coordination, and follow-up.
- Partnering with interdisciplinary team members and supporting shared decision-making with patients and families.
- Making clinical decisions regarding in-office management versus specialist referral.
- Handling inbox responsibilities such as test results, messaging, medication management, and referrals.
- Supporting practice efficiency and quality initiatives.
Qualifications include:
- Master’s degree and national certification (ANCC, AANP, or NCCPA).
- Active, unrestricted Massachusetts NP or PA license, DEA, and BLS certification.
- Minimum of three years of independent primary care experience, including preventive, longitudinal, and chronic disease management, especially for adult patients aged 65+.
Preferred skills:
- Experience in value-based, population health, or team-based care models.
- Experience managing complex adult patients with multiple conditions.
- Familiarity with clinical pathways, quality initiatives, and Epic EHR.
