Role Overview:
The Wound Care APC provides comprehensive medical, diagnostic, therapeutic, and preventive care for patients with chronic and complex wounds or edema, in outpatient and home settings. Responsibilities include evaluating and treating wounds, monitoring healing progress, educating patients and families on wound care outside clinical settings, and ensuring care aligns with evidence-based guidelines and regulatory standards. The role involves advocating for patients, utilizing managed care processes for cost efficiency, documenting in EMR systems, collaborating with healthcare teams, and conducting educational activities for staff and patients.
