Role: HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) responsible for conducting in-home health assessments for Medicare Advantage and other plan members.
Responsibilities: Perform comprehensive assessments including medical history, medication reconciliation, vital signs, physical exams, screenings, and point-of-care tests. Identify and document diagnoses, communicate findings to primary care providers, recognize urgent situations, educate members on health management, and address social determinants of health with referrals as needed.
Qualifications & Skills: Active, unrestricted licensure and certification (ANCC, AANP, NCCPA), valid driver’s license, reliable transportation, ability to navigate home environments, and proficiency with electronic health records. Preferred: 1+ years of clinical experience, strong communication skills, and ability to handle equipment and stairs.
