Role Overview:
As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care and health education. Your responsibilities include reviewing medical history, performing physical exams, screenings, and point-of-care tests, identifying diagnoses, and communicating findings to primary care providers. You will recognize urgent situations, provide patient education on disease management, medications, and social determinants of health, and offer necessary referrals.
