Role Overview:
As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform comprehensive in-home health assessments for Medicare Advantage members, focusing on improving health outcomes through education, gap closure, and collaboration with primary care providers. The role involves conducting physical exams, reviewing medical history, medication reconciliation, screenings, and identifying urgent issues. You will communicate findings to PCPs, address social determinants of health, and provide patient education.
Key Responsibilities:
- Perform in-home assessments, including vital signs and screenings
- Document diagnoses and assist in care management planning
- Collaborate with PCPs and recognize urgent/emergent situations
- Educate patients on health conditions and medications
- Address social factors affecting health and provide referrals
Qualifications & Skills:
- Active, unrestricted licensure and certification (ANCC, AANP, NCCPA) in relevant fields
- Reliable transportation and ability to navigate home environments
- Strong communication skills, with experience working with geriatric or Medicare populations preferred
- Proficiency with electronic medical records and technology
- Ability to lift equipment up to 30 pounds and navigate stairs
Compensation & Benefits:
Salary ranges from $125,000 to $175,000, including base pay and bonuses, with benefits such as health coverage, incentives, 401(k), and stock options. The role offers flexible scheduling, no on-call or weekend work, and an environment that supports professional growth.
