We are seeking a full-time Case Manager to support our Emergency Department team, focusing on discharge planning, patient resource connection, and promoting positive outcomes.
**Key responsibilities include:**
Coordinating care from pre-admission to post-discharge, Facilitating interdisciplinary communication and early intervention, Evaluating outcomes using clinical, financial, and utilization data, Advocating for patients to resolve issues within healthcare delivery, Connecting patients with community resources, and Supporting families through crisis intervention and education.**Qualifications include:**
Bachelor’s Degree in nursing or Social Work, Unencumbered RN license (for Care Managers) or licensed Social Worker credential, Basic Life Support (BLS) certification within six weeks (for Social Workers), Strong communication, critical thinking, and problem-solving skills, Ability to work in diverse healthcare settings and collaborate with multidisciplinary teams.
