Role: Licensed Practical Nurse (LPN) providing professional nursing care in long-term care settings under supervision.
Responsibilities:
- Deliver nursing care tailored to residents’ physical and psychosocial needs, following care plans.
- Perform assessments, administer medications and treatments, and evaluate resident data.
- Recognize problems, recommend solutions, and respond effectively to health emergencies.
- Document resident interactions, care outcomes, and maintain accurate records.
- Educate residents and families on prevention and self-care programs.
- Collaborate with healthcare team members and support interdisciplinary care.
- Maintain confidentiality and communicate clearly with residents, families, and staff.
- Perform additional duties as assigned, including technical support and resident counseling.
Qualifications & Skills:
- Graduate from an accredited practical nursing program; associate degree preferred.
- Current, unencumbered LPN license with state and/or multi-state privileges.
- Experience or coursework in IV therapy (if required for the role).
- Ability to perform clinical assessments, work effectively with teams, and maintain confidentiality.
- Willingness to complete continuing education and department-specific certifications.
