Role: The Physical Therapist provides primary rehabilitative care to 25-35 patients in a home setting, coordinating multidisciplinary services and supervising PTAs and Home Health Aides.
Responsibilities: Assessing and managing patient needs, developing and adjusting plans of care, performing routine and complex therapy procedures, and ensuring positive outcomes. Facilitates patient and caregiver education to promote independence. Maintains communication with physicians and team members, documents accurately, and participates in performance improvement activities. Ensures compliance with regulatory guidelines and completes all required documentation timely.
Qualifications: Graduate of an accredited physical therapy program, licensed as a Physical Therapist, with at least 1 year of full-time experience, preferably in home health. Certifications include BLS Instructor, valid driver’s license, and private vehicle insurance.
Skills: Strong assessment, rehabilitation, leadership, problem-solving, organizational, and communication skills. Proficiency in home health operations, regulatory processes, and OASIS knowledge is preferred.
