Licensed Field Care Coordinator – Middle TN

Role Overview: The Care Coordinator applies clinical knowledge to manage the care of medically complex members, ensuring access to appropriate services and provider coordination. This field-based position involves early risk identification, patient education, self-management support, and developing individualized care plans in collaboration with members, families, and providers.

Key Responsibilities:

Coordinate member care across settings, including acute, home, and chronic care, using evidence-based practices

Conduct assessments, facilitate group visits, and educate members on health management and equipment use

Serve as advocate, resolve care barriers, and connect members to community resources

Monitor hospital encounters, coordinate with inpatient teams, and ensure smooth care transitions

Document care activities, participate in quality improvement, and utilize health plan software

Collaborate with the care team to improve care quality and efficiency

Qualifications & Skills:

Current RN or Social Worker license with ≥2 years clinical experience
Experience with MS Office and strong organizational, communication, and time management skills
Valid driver’s license, reliable transportation, and ability to travel within assigned territory
Preferred: BSN, Master’s degree, CCM certification, community case management or long-term care experience, and knowledge of Medicare/Medicaid regulations
