Qualifications & Skills:
- Current RN or Social Worker license with ≥2 years clinical experience
- Experience with MS Office and strong organizational, communication, and time management skills
- Valid driver’s license, reliable transportation, and ability to travel within assigned territory
- Preferred: BSN, Master’s degree, CCM certification, community case management or long-term care experience, and knowledge of Medicare/Medicaid regulations
