Licensed Field Care Coordinator – Middle TN

Role Overview: The Care Coordinator applies clinical knowledge to manage the care of medically complex members, ensuring access to appropriate services and provider coordination. This field-based position involves early risk identification, patient education, self-management support, and developing individualized care plans in collaboration with members, families, and providers.

Key Responsibilities:

  • Coordinate member care across settings, including acute, home, and chronic care, using evidence-based practices
  • Conduct assessments, facilitate group visits, and educate members on health management and equipment use
  • Serve as advocate, resolve care barriers, and connect members to community resources
  • Monitor hospital encounters, coordinate with inpatient teams, and ensure smooth care transitions
  • Document care activities, participate in quality improvement, and utilize health plan software
  • Collaborate with the care team to improve care quality and efficiency

  • Qualifications & Skills:

    • Current RN or Social Worker license with ≥2 years clinical experience
    • Experience with MS Office and strong organizational, communication, and time management skills
    • Valid driver’s license, reliable transportation, and ability to travel within assigned territory
    • Preferred: BSN, Master’s degree, CCM certification, community case management or long-term care experience, and knowledge of Medicare/Medicaid regulations
