Position Overview
Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions aligned with physician directives within a home health setting. Responsibilities include conducting initial and ongoing assessments of mobility, strength, and function, documenting evaluation and treatment plans, and adjusting goals as needed. The OT provides therapy services, collaborates with healthcare team members, and educates patients and caregivers on discharge plans and home exercises. Additional duties involve maintaining accurate documentation, participating in service coordination, supervising personnel, and completing assessments like OASIS where permitted. Candidates should possess an accredited degree in Occupational Therapy, a valid state license, CPR certification, and a driver’s license. Preferred skills include strong organization, communication, attention to detail, and experience with home health therapy and OASIS documentation.
