Position Overview
The Physical Therapist assesses, teaches, and delivers therapy interventions based on physician-directed home health plans. They collaborate with an interdisciplinary team, providing updates to optimize patient well-being.
Key Responsibilities:
- Perform assessments of mobility, strength, and function.
- Administer, evaluate, and interpret tests to set functional goals.
- Develop and update plans of care, documenting progress and outcomes.
- Deliver therapy services according to physician orders.
- Coordinate discharge planning and educate patients and caretakers on home exercises.
- Collaborate with healthcare team members, document effectively, and supervise personnel if applicable.
- Complete assessments like OASIS where permitted.
Qualifications & Skills:
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license.
- Proficiency in assessments, documentation, and policy adherence.
Preferred Attributes:
- Strong organization, communication, and attention to detail.
- Effective time management and relationship-building skills.
- Experience in home health therapy and OASIS documentation is a plus.
