Position Overview
Seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover at home. Part-time weekly visits in Seminole County with weekend on-call rotation.
Key responsibilities include:
- Conduct initial and ongoing assessments of mobility, strength, and function.
- Administer, evaluate, and interpret tests to set functional goals.
- Provide detailed evaluations and care plans outlining needs, goals, and outcomes.
- Deliver therapy services according to physician orders and document appropriately.
- Maintain and modify the plan of care, and develop discharge plans with patient education.
- Collaborate with healthcare team members and document effectively to ensure compliance and avoid denials.
- Supervise other personnel (PTA, HHA) as applicable and complete OASIS assessments where permitted.
Qualifications include:
- Degree in Physical Therapy from an accredited program.
- Unrestricted state license as a PT.
- Valid CPR, driver’s license, and proof of insurance.
Preferred experience: Home health therapy and OASIS documentation.
