Position Overview: A licensed Physical Therapist (PT) provides essential home health care services, enabling patients to recover from illness or injury at home.
Key Responsibilities:
- Conduct initial and ongoing assessments of mobility, strength, and function.
- Develop and interpret tests to set functional goals and outcomes.
- Prepare detailed evaluation and care plans, including progress and discharge strategies.
- Deliver therapy services per physician orders, documenting care and modifying goals as needed.
- Coordinate with healthcare team, educate patients and caregivers on home exercises, and participate in discharge planning.
- Supervise staff (PTA, HHA) as applicable and complete OASIS assessments within scope of practice.
- Ensure documentation compliance to prevent claim denials and adhere to all policies, standards, and legal requirements.
Qualifications & Skills:
- Degree in Physical Therapy from an accredited institution.
- Current, unrestricted state license.
- CPR certification, valid driver’s license, and proof of auto insurance.
- Preferred: Experience in home health therapy and OASIS documentation.
