Position Overview
An Occupational Therapist in home health provides personalized, one-on-one care to help patients live independently and safely in their homes. Responsibilities include conducting initial and ongoing assessments of mobility, strength, and function, and developing comprehensive plans of care with clear goals and expected outcomes. The role involves delivering occupational therapy services per physician orders, documenting progress, and modifying treatment plans as needed. The therapist collaborates with healthcare team members, educates patients and caregivers, and participates in discharge planning. Supervision of other personnel and completion of OASIS assessments are part of the duties. Requirements include a degree in Occupational Therapy from an accredited program, a current state license, valid CPR, and a driver’s license.
