Role Overview: A licensed Physical Therapist (PT) provides in-home physical therapy services to patients, helping them recover from illness or injury within their own homes. The role involves conducting assessments, developing and modifying plans of care, and delivering prescribed therapy to meet patient goals.
Key Responsibilities:
- Perform initial and ongoing functional assessments
- Develop and document treatment plans, goals, and progress
- Administer therapy services in accordance with physician orders
- Coordinate with healthcare teams, patients, and caretakers
- Supervise relevant personnel such as PT Assistants and Home Health Aides
- Complete required documentation, including OASIS assessments where applicable
- Ensure compliance with all care standards, regulations, and policies
Qualifications & Skills:
- Degree in Physical Therapy from an accredited program
- Current, unrestricted state licensure
- Valid CPR, driver’s license, and automobile insurance
- Preferred: Home health therapy experience and OASIS documentation skills
