Role Overview:
This position provides PRN care management support, focusing on advocating for patients’ health needs, addressing barriers to care, and developing interdisciplinary care plans.
Key Responsibilities:
Assess and plan for patients with complex medical conditions, ensuring safe transitions between care levels.
Utilize telehealth, home, or field-based settings to deliver care and collaborate with interdisciplinary teams.
Engage patients and families to identify goals, support functional independence, and address care gaps.
Leverage community resources, ensure legal and ethical compliance, and document activities per standards.
Work independently within scope, demonstrating strong communication, assessment, and problem-solving skills.
Qualifications & Skills:
Current RN license with a BSN or higher degree.
Preferred CCM or RN-BC certification, attainable within 36 months.
Excellent interpersonal, verbal, and written communication skills.
Proficiency in Microsoft Office and understanding of managed care principles.
Ability to manage multiple tasks in a fast-paced, evolving environment with critical thinking skills.
