Requirements:
- Minimum of 2+ years of relevant experience
Key Responsibilities:
- Perform reassessments and update care plans based on patient progress
- Communicate functional changes and barriers to the care team for effective planning
- Educate patients and caregivers on exercises, safety strategies, and home programs
Work Details:
- Manage 8-hour shifts, totaling 40 hours weekly
